DHCF is updating Transmittals 17-02 and 17-02(R), and the summary notice originally issued on February 1, 2017 to reflect the new mailing address for Terris, Pravlik and Millian, LLP as of August 28, 2017. In addition, the Civil Action Number was changed to reflect the correct case number. No other information has changed. Effective immediately, please use this updated transmittal and summary notice to inform beneficiaries of the procedure for requesting reimbursement for out-of-pocket expenses.
