DHCF is updating Transmittal 17-03 and the summary notice originally issued on February 1, 2017 to reflect the new mailing address for Terris, Pravlik and Millian, LLP as of August 28, 2017. No other information has changed. Effective immediately, please use this updated transmittal and summary notice to inform beneficiaries of the procedure for requesting reimbursement for out-of-pocket expenses.
