After further assessment, the Department of Health Care  Finance (DHCF) has decided to cancel provider reenrollment. In place of reenrollment, DHCF is requesting that all providers complete the Provider Demographic Information Update Form. This pre-populated form will be mailed to all providers in our system along with a copy of the transmittal.
