As part of our broader effort to clarify and streamline the Department of Health Care Finance (DHCF) policies for providers, DHCF has revised the Form 1728 – Request for Medicaid Nursing Facility Level of Care.  As a reminder, Medicaid beneficiaries must meet a nursing facility level of care (LOC) in order to receive long-term care services in a nursing facility or through the EPD waiver program.
