The last paragraph on page 3 changes the contact person’s name/information from Yvonne Iscandari, Program Manager, Division of Long Term Care to Robin Exton, Project Manager, Elders and Persons with Physical Disabilities Branch, Division of Long Term Care. This Transmittal renders Transmittal 12-08 obsolete, effective immediately.
