In order to access long term care services and supports, individuals must apply for Medicaid using a special application for long term care. In an effort to streamline the Long Term Care Application submission process, the Department of Health Care Finance has developed a new revised Long Term Care (LTC) Application for applicants and enrollees to use at initial application and renewals of Medicaid coverage. The new application also means that individuals no longer will have to submit a Combined Application in addition the LTC Application. This transmittal describes new requirements and processes for using the new LTC Application.
