DHCF has released an update to the person–centered planning template. This update includes changes required to be in compliance with the federal 1915(c) Home and Community-Based Services (HCBS) Waivers Final Rule, published January 2014, and reflects other programmatic changes, including additional fields specific to the Services My Way Program. All providers are required to use the new template as of Friday, April 8, 2016.
