PACE4DC opened its doors to District of Columbia participants in March 2023!
The Program of All-Inclusive Care for the Elderly (PACE) is a comprehensive health and long‑term care program designed to help eligible older adults remain safely in their homes and communities for as long as possible.
What is PACE? Who is eligible?
PACE is a nationally recognized model of care that integrates Medicare and Medicaid benefits for eligible beneficiaries. Under this model, participants receive a broad array of services beyond those typically available under either program. Their care is coordinated and managed by a comprehensive, interdisciplinary team dedicated to delivering high-quality, well-coordinated care.
- To be eligible for PACE, individuals must:
Be age 55 or older
Meet the nursing facility level of care criteria
Reside in the PACE4DC service area, limited to three ZIP codes in Wards 7 and 8 (20019, 20020, and 20032)
Who is a good candidate for PACE?
Individuals who meet all enrollment criteria who live in Wards 7 or 8, who are age 55 or older, and meet the level of care criteria may be strong candidates for PACE. To learn more about what PACE offers, explore these resources:
