Digital Health Roadmap

Health Care Reform and Innovation Administration (HCRIA)

The District State Medicaid Health IT Plan (SMHP) serves as the DHCF’s 5-year roadmap for leveraging digital health to connect residents and providers, ensuring health information is accessible when and where it’s needed.

Initially published in 2018 and updated in 2022, the SMHP serves as the District’s Digital Health Roadmap and guides our initiatives, including DC HIE infrastructure development and use case implementation that support the following priority areas:

  • Transitions of Care for Individuals
  • Social Determinants of Health Data
  • Population Health Management
  • Public Health
  • Behavioral Health Transformation
  • Telehealth

Developed through rigorous interviews, focus groups, and an environmental scan, this document represents stakeholder-identified priority areas for how digital health can improve patient and provider experience in the District.

2022 SMHP Update

The 2022 SMHP Update reviews progress made since the 2018 State Medicaid Health IT Plan and addresses current and future digital health needs in Medicaid program.

The 2022 SMHP Update outlines seven main recommendations derived from interviews and focus groups with participants who expressed their views on addressing challenges and enhancing the DC HIE. These recommendations aim to guide DHCF and the District in achieving better health system outcomes.

  1. Develop and Publish a Bi-Annual Evaluation and Strategic Plan, including Metrics to Effectively Assess Digital Health Impact
  2. Broaden and Deepen Investment in the DC HIE through Interagency Collaboration to Address Technology Gaps, Build Digital Health Capacity, and Support the Long-Term Sustainability of the DC HIE
  3. Invest in District-wide Population Health Analytics, including Access to Priority Data
  4. Engage Community-Based Organizations and Facilitate Partnerships with Clinical Providers to Expand Access and Use of Social Needs Information in the DC HIE
  5. Enhance the DC HIE Consumer Experience, for both Providers and Patients
  6. Improve Education and Communication to Increase Awareness of the DC HIE
  7. Develop and Promote Payment Models and Provider Incentives to Drive Adoption and Use of the DC HIE
