State Innovation Model

The Centers for Medicare & Medicaid Services (CMS) is collaborating with states and territories to support the design and implementation of new health service delivery and payment methods through the State Innovation Models (SIM) Initiative. In 2015, CMS selected the District of Columbia for a SIM Design Award of $999,998. Over the next year, DHCF will engage a diverse group of both private and public stakeholders, including payers, providers and consumers of health care services to develop a State Health Innovation Plan for the District of Columbia. The overall goal of the plan will be to bolster primary health care and to better coordinate the care of the District’s most vulnerable resident, in order to:

  1. Improve the health of the District’s population;
  2. Enhance the patient experience of care; and, 
  3. Reduce or at least control healthcare costs in the District.

 

Why does the District want to improve how health services are delivered now?

Residents of the District of Columbia have one of the highest health care coverage rates in the nation. Despite the District’s substantial investment in health care, huge disparities exist between the health outcomes of many District residents. The SIM Award will allow us to take a step back and collectively determine the best strategy to improve the health of all District residents.
