DHCF is in the process of overhauling our case management services.  This includes adjustments to the service definition of case management, implementation of conflict-free case management and person-centered planning principles, a new per-member-per-month payment methodology, and other vital changes. Our intention is to strengthen the services of our current case managers and also to enroll new case management providers with experience in Long Term Care. 

To that end, we are inviting all interested prospective case management agencies to meet with key staff at DHCF to learn more about providing case management services to beneficiaries of the Elderly and Persons with Disabilities (EPD) 1915c Waiver Program. These 90 minute sessions (July 13, 11:30 am  - 1 pm; July 17, 11 am - 12:30 pm) will provide an overview of the following, as well as an opportunity to ask questions of DHCF staff:

• Changes to EPD Waiver Program (including changes to case management services and payment methodology)
• Medicaid provider enrollment
• Billing and financial management
